1. Complete your LT Tryout Registration Form.
2. Fill out the USAV Waiver of Liability.
3. Fill out the Concussion & Sudden Cardiac Arrest Acknowledgement.
4. LT Liability Waiver
5. Pay for your tryout fee. $25
6. Pay for your Tryout Membership through Sports Engine Please note: If you had a Sports Engine account for 2020-21 season, use the same login.
** You cannot participate without a valid USAV Tryout Membership.
✓ Medical Release waiver
✓ Concussion and Sudden Cardiac Arrest form
